 How To Search:     Explanation
Search by Name In the appropriate box (last name, first name), enter any part of the name you wish to find.  To avoid returning too many names, use at least several letters.  If you do not find a record, try shortening the search to a few letters you are sure would be in the name.  If you wish to find someone in a specific profession, make a selection in the third box.
Search by Company Name Enter any part of the name of the company.  To avoid returning too many company names, use at least one word.  If you do not find a record, try shortening the search to a few letters you are sure would be in the name.  If you wish to limit the search to specific company type, make a selection in the second box.  If you want only companies with a specific status, make a selection in the third box.
Search by License Number Enter all five numbers in the license number, including leading zeros.  If more than one result is returned, it is because there are two different types of licenses with the same number.  In the data system, all license numbers include a license type identifier as part of the license, such as TMP for temporary license and PMD for permanent medical doctor.
Search by License Type First select the license type and the license status you wish to view.  If you are selecting a permanent license type, the search could return thousands of results and take a lot of time.  Select a county to narrow your search.
Search by Specialty Physicians and podiatrists who have identified a primary or secondary specialty will be listed when you select the specialty name from the drop-down box.  Select a status category and/or a county to narrow your search.  The county is the primary work location but is usually only available for active licensees.
Search by Work Location Select the county and one profession to find licensees who have a current work address listed in the selected county.

Print a Record Click Printer Friendly to view a copy of the licensee's record that is formatted for printing.
Find Licensee in Other States Click any state listed in the licensees' record of other states where licensed.  If the state has a website or contracts for another entity to display the physician record, a new window will open where you may search for information in that state.
View Discipline Records If the licensee has any disciplinary records that are public information, the number of cases will be listed. To view the public data, click that link and a detailed page will open.

 Type of License or Registration     Description
License and Registration Types Each license or registration issued by the WV Board of Medicine to the individual is listed, along with license status, the original issue date, and if applicable, the expiration date.
Permanent License A permanent license is a full license to practice at any location in West Virginia and at any time, as long as the license is active (see 'Status' below).  A permanent license must be renewed every two years.
Children's Summer Camp License A children's summer camp license is issued to allow physicians or podiatrists to serve for a limited duration as staff in camps located in West Virginia.
Medical School Faculty A physician may obtain a one-year medical school faculty license to practice medicine at the medical center of a medical school in West Virginia.
Volunteer A one-year volunteer license may be issued to a qualified medical doctor to serve in a charitable clinic as a volunteer.
Drug Dispensing Certificate A licensed medical doctor or podiatrist may apply for authorization to dispense drugs.  A separate certificate is required for each location (e.g. office or clinic) where drugs are dispensed.  Drug dispensing is not the same as drug prescribing.
Prescriptive Writing Privilege A physician assistant may apply for authority to write prescriptions (for specific classes of drugs only) under the direction of his or her supervising physician.
Medical Corporation A medical corporation is a group of physicians or podiatrists who practice medicine and manage their business functions as a corporation that has been authorized by the WV Board of Medicine and chartered by the West Virginia Secretary of State.
Professional Limited Liability Company (PLLC) A PLLC is a group of physicians who practice medicine and manage their business functions as a PLLC that has been authorized by the WV Board of Medicine and chartered by the West Virginia Secretary of State.
 Status:     Description
License Status Terms:   Not all statuses are assigned to every license type.
Active The Active license or registration is current and in good standing and the individual or company is fully authorized to practice in accordance with the specific authorization.
Inactive An Inactive status is available only to physicians and podiatrists, who may keep the option of returning to active status by maintaining an inactive status and continuing to complete continuing education.  An inactive status licensee may not practice medicine/podiatry in any manner whatsoever in the State of West Virginia.
Lapsed A Lapsed status is available only to physicians and podiatrists.
Expired A drug dispensing certificate expires when the term of registration ends without renewal having been completed.

These licenses expire at the end of the term: Temporary License, Children's Summer Camp License.
Suspended A status of Suspended means the licensee is unauthorized to practice for a certain term due to disciplinary action.
Suspended No CME A status of Suspended No CME is automatically assigned when the physician or podiatrist fails to renew with proof of adequate continuing education.
Surrendered A licensee who agrees to give up his or her license to practice surrenders the license. This may be by order with the WV Board of Medicine.
Revoked The WV Board of Medicine may revoke a license to practice.
Terminated The status of terminated is assigned to physician assistants who do not renew a license when the period of registration ends or when they are no longer practicing, as well as to Medical Corporations and PLLC’s which are not renewed at the end of the period of registration.

Requesting a Correction If you are licensed with the WV Board of Medicine and your record contains an error that can be corrected by referring to existing records, please e-mail your request.
Requesting a Change If you are licensed with the WV Board of Medicine and you wish to update your record with new information, please submit the new information in writing to:
